Christ Lutheran Church Costa Rica Mission Trip Team Registration
Application and $150 deposit due by November 24. The Team will be formed by December 1. You may register as a traveler, funder, supporter, or class attendant (or multiple). All travelers must attend the pre-departure classes, and anyone that signs up to participate in the class must commit to attending at least 6/8 meetings. One registration must be filled out for each individual (not per family).

Team members and class attendants will be required to attend 7 of 8 (or all 8!) predeparture classes, approximately once per month from December 2019-June 2020. Dates TBD. The tentative trip dates are June 12-19, 2020.

The total cost for the trip is $2,200 per person. This includes transportation to/from the airport, airfare (based on $600/person estimate), in-country transportation, room (in guesthouse), 2 meals per day in-country, bilingual staff 24/7, basic materials ($1,500), and a basic tourism day. This does not include: health insurance, any visa related fees, 1 meal per day, snacks and other tourism in-country. Half of this ($1,100) is due by January 31, 2020, and the full balance is due by March 30, 2020.

If I cancel my trip for personal reasons or if I fail to comply with any international travel regulations (such as valid passport) and ask for a refund, I accept the following refund policies (The team will do our best to find a replacement to take your place and refund you all of your payments. If this is not possible, the following applies):
a. A refund is available up to 30 days before the trip (May 12, 2020), with a $500 administration fee withheld
b.29-15 days prior to the service trip, a refund can be made to participants, but with an $800 administrative cancellation fee.
c.7 days or less prior to the trip, no refunds can be made to participants. A maximum of $1000 can be applied to another PRAXIS service trip.

I also understand that if I desire a full refund, I will secure proper travel cancellation insurance.
