I agree to abide by the safety protocols put forth by Mindful Kids Stratford & Mindful Kids Inc.

I am fully aware of the risks and hazards inherent in my child's attendance at any Mindful Kids Stratford location and participation in the services provided by Mindful Kids Stratford.

I freely and voluntarily agree to assume the risk, regardless of severity, that my child may sustain as a result of attendance at any location of Mindful Kids Stratford, however arising.

I permit my child to attend Mindful Kids Summer Camp or any service provided by Mindful Kids Stratford, I the undersigned, permit my child/ren to participate in the full range of camp activities and authorize the Camp Director and her appointee, in the event of the emergency affecting your child/re to authorize, on my behalf, all procedures, including admission to the hospital and necessary treatment therein, as he/she may deem essential for care and wellbeing of the participant. Such action is only to be taken when immediate contact with the undersigned cannot be made. I understand that pictures may be taken & used as promotion or posted online via Mindful Kids Stratford social media outlets. I fully understand the risks involved in the above named’s participation in Mindful Kids Stratford's programs and accept full liability. I also grant permission for my child to participate in any excursions Mindful Kids Stratford will participate in.
