Hello Class of 2022!

This form is intended for me to learn more about you and your plans for after high school. If you don't plan to go to college, or if you're not sure, please still fill this out, because it's a great way for me to help you with whatever plan you might come up with! We will follow up in-person or via zoom once I receive your responses.

Please take your time, and give these questions some thought - it's not meant to be completed in 5 minutes. I really appreciate you doing this! It means a lot to me!

School activities: List the activities in which you have participated, the number of years for each, and the amount of time per week you spent. What have you gained or learned from each activity? Consider artistic, athletic, literary, community service and leadership positions. Which activity was most important to you, and why? *
Other activities: What do you consider your most important activities outside of school? List jobs, paid or voluntary; religious activities; outside sports; hobbies; clubs; travel; music; art, etc. *
Awards/Honors: Which awards or honors have you received, and when (during high school years only, please)? What were they awarded for? Please include academic, athletic, and/or community honors. Example: I received the Coach's Award in 10th grade Tennis because I was recognized for always helping my teammates, was always the first one to practice and improved my skills dramatically over the season.
