Fees:

Proximity Card: $20.00
Lost Permit: $35.00

A confirmation email will be sent once the request has been processed. Please allow 24-48 hours for the request to be processed. Weekends and holidays may take longer.

Replacement permits and Proximity cards are available for pick-up at Student Services in Lyons hall (room 103).

If your permit decal or proximity card is defective, please bring it to Student Services for replacement at no cost.

Permit holder is responsible for all fees.
