PUMP-OUT REGISTRATION

ON THE HOOK MARINE SERVICES LLC. Provides mobile vessel pumpout service throughout Monroe County. We are contracted by FDEP to perform free pumpouts for qualified vessels in approved anchorages. We also provide private pumpout service for fee. WE MAY SHARE YOUR INFORMATION WITH MONROE COUNTY, FLORIDA, THE CITY OF KEY WEST, FLORIDA, AND/OR THE STATE OF FLORIDA. If your vessel is not in an approved anchorage found on our website, we will charge you a fee to provide this service.

I acknowledge that standard service fees will apply, that I am responsible for providing accurate contact and location information, and that payment will be required in accordance with the provider’s current rates and billing policies. By submitting this form, I agree to be contacted to schedule service and authorize the provider to bill me for any pumpouts I request or receive under this private pay registration.

Registration Acknowledgment:

• I understand that it is my responsibility to ensure this vessel and all waste valves are in the proper position for waste collection.

• I understand that I will not receive mobile pumpout service unless I display the orange pumpout flag indicating a request for pumpout services.

• I understand that weather and other scheduling and health or safety considerations may impact receiving pumpout services.

• I understand that the Department of Environmental Protection, Monroe County and On the Hook Marine Services will not for any reason in connection with or in relation to the provision of the service be responsible or liable for any damages including but not limited to vessel defects or improper installation of holding tank systems.

• I understand that the Department of Environmental Protection, Monroe County and On the Hook Marine Service’s service personnel may need to board this vessel, if necessary, to perform the service.

• Service will not be provided to a vessel that does not have sewage to be pumped, and I will not receive a monthly endorsement sticker if services have not been provided.

• Service may be denied for any reason including but not limited to if the vessel is not properly equipped to provide for a safe, secure pumpout as determined by the pumpout vessel operator.

• By executing this Registration Form, I attest that I am legally authorized to authorize provision of the service by the Department of Environmental Protection, Monroe County and On the Hook Marine Service , and that I assume all liability, and shall indemnify and hold harmless the Department of Environmental Protection, Monroe County and On the Hook Marine Service for the provision of the service pursuant to their reliance upon the truth and accuracy of this executed Registration Form.

Thank you for registering! If you're eligible for FREE pump-out, we are required to share your information with the Florida Department of Environmental Protection and Monroe County, Florida. We may also share some information with the City of Key West. If you're not eligible, we may share some or all of your information for data purposes relating to the Clean Vessel Act. Otherwise, we will not share any identifying information with anyone except our payment processor. *
