The purpose of this survey is for students to be able to give feedback on their teacher's performance. Each page has 25 questions for each teacher.
Only answer a page per teacher. For example, if you have a teacher for science and then have them in a science rotation class too you will just fill out 1 page for that 1 teacher. So if you have Mrs. Gordon for 7th grade science and then also have her for science rotation you will only fill out 1 page on Mrs. Gordon.
