1. Assumption of Risk & Release of Liability

I acknowledge that participation in art-related activities, sensory play, and play-based activities at Art Barn Mill Valley involves inherent risks. These may include, but are not limited to: use of art supplies (paints, markers, scissors, adhesives, glues); sensory materials (clay, sand, textured objects); falls or slips on the premises; falls from or injuries involving the play structure, climbing wall, or other equipment; and risks associated with outdoor play in Art Barn Mill Valley’s outdoor play space; collisions with structures, equipment, or other participants; and allergic reactions, choking, or injuries related to materials or activities.

I understand these activities carry the risk of injury or, in rare cases, serious harm. I voluntarily choose to allow my child to participate and accept all associated risks.

To the fullest extent permitted by law, I release and discharge Art Barn Mill Valley, its owners, employees, instructors, volunteers, affiliates, and the property owner/landlord of the premises from all claims or causes of action arising from any accident, injury, illness, or property damage occurring during participation, whether caused by negligence or otherwise.

2. Consent to Participate

I grant permission for my child to participate in all art, play, and sensory activities offered by Art Barn Mill Valley, including off-site activities if applicable. I believe these activities are appropriate for my child’s age and development.

3. Health & Medical Emergency Authorization

I acknowledge that Art Barn Mill Valley does not provide medical services. In the event of an emergency, I authorize staff to seek medical treatment for my child. All reasonable efforts will be made to contact me or my emergency contact as quickly as possible. I certify that my child is physically able to participate and has not been advised otherwise by a medical professional.

4. Behavioral Expectations

I understand that all participants must follow established rules, respect others, and maintain a safe environment. If my child’s behavior becomes unsafe or disruptive, Art Barn Mill Valley may require my child to be removed from the activity, and I agree to pick up my child immediately if requested.

5. Parent/Guardian Responsibility & Supervision

I understand and agree that adult supervision of each child by their parent or authorized caregiver is required at all times while at Art Barn Mill Valley. During Art Barn parties, events and classes where caregiver presence is required, parents are required to supervise closely and ensure children follow all safety rules at all times. Art Barn Mill Valley is not liable for injuries resulting from lack of supervision, including a child leaving the premises without a parent or caregiver. Even while Art Barn staff is present,, the ultimate responsibility for my child’s safety, behavior, and conduct remains with the parent/guardian at all times. Party hosts are responsible for any children attending parties whose parents are not present.

6. Agreement to Follow Posted Rules

I agree to follow all rules posted at the facility or communicated by staff. If I or my child observe a hazard or injury, I will promptly notify a staff member and consent to any injuries being photographed and, if appropriate, treated with basic first aid.

7. Multi-Use Waiver Acknowledgment

I understand that this waiver will remain in effect for future visits and participation unless I revoke it in writing. This waiver may also apply to my other child/children as permitted.

8. Indemnification

I agree to indemnify and hold harmless Art Barn Mill Valley, its owners, employees, instructors, volunteers, affiliates, and the property owner/landlord of the premises from any loss, liability, damage, or costs (including attorney fees) arising from participation in activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

9. Media & Photography Release

I grant permission for Art Barn Mill Valley to photograph or video my child during activities. These images may be used for promotional purposes, including social media, the website, and printed materials. I understand that no compensation is provided for such use.
