Questionnaire for the cat's guardian before the live consultation
Before the consultation, please fill in the data below, which is necessary to start the diagnosis and prepare a treatment plan and additional materials for our meeting. Thanks to this, it will be easier to plan the best consultation form and the time we might need for our meeting. Knowing the problem in more detail, I might ask you to prepare some videos or perform a veterinary check-up before the consultation. Filling in all the information below now will allow us not to waste any precious time of the meeting itself on formalities and setting up a client card. We can focus 100% on improving the comfort of your cat and you.

NOTE -  If you have several cats, of course. In each of the questions below, tell us about each of them. Even if the question, for simplicity, is in the form of a question about only one cat. Thank you!
