In consideration of the Sparta Youth Football camp, I am allowing my child to participate in its Football camp, I understand that my child must have current and active medical insurance before my child can attend camp. I hereby register my child and authorize the staff to direct my child's participation in camp activities. My child has no medical or emotional problems, which may affect my child's ability to safely participate in your program. In the event of injury, I authorize the coaches and athletic training staff to obtain and/or administer any medical care or treatment deemed necessary. I understand that there are inherent risks associated with sports activity and that neither I nor my child will hold the school or coaches liable for any injuries sustained at the camp.  The Sparta Youth Football camp, its coaches, school, and all its employees, acting officially or otherwise are hereby released from any and all claims, demands, actions, or causes of action on account of any injury to my child that may occur.  I understand that the Sparta Youth Football camp may, in its sole discretion, dismiss any camp participant for inappropriate, disrespectful, or dangerous behavior at any time. In this event, I understand that I will not receive a refund of camp fees for unattended days.  If my child breaks or damages any property as a result of their direct or indirect behavior, I hereby agree to pay for its repair or replacement.  Additionally, I give my permission to utilize any video or photos that may include my child or myself for any commercial use to promote the camp.
