國立臺灣大學語文中心中國語文組教學評鑑   CLD Chinese Course Evaluation
1. 這份調查表是為了幫助我們改進課程而設計的，安排在期末進行，是為了希望能即時發現問題，即早解決。
2. 請您放心，評鑑結果學期結束才會給老師，這個學期間老師完全不會看到任何關於評鑑的內容。
3. 雖然我們請您填了學號，但只有承辦的行政人員才看得到，您的身份會完全保密，老師絕對不會知道您寫了什麼。
4. 為提升教學品質，CLD相當重視您的意見，並如實反映給老師和行政人員，惟請勿人身攻擊並避免使用情緒性字眼，以免造成爭議。

1. This questionnaire is designed for us to improve the quality of courses. The reason why it is done at End of term is because, if there is any problem, we want to know it and solve it before it is too late.
2. Please do not worry, this evaluation will not be given to the teacher until this season is finished. Your teacher will not know anything about this evaluation during this season.
3. Though you need to fill in the ID, it is only for the staff who is responsible for this evaluation. No one else (including your teacher) will know who fills this evaluation. Your identity is completely anonymous and confidential.
4. To enhance the quality of teaching, the CLD highly values your feedback and will faithfully convey it to instructors and administrative staff. However, please refrain from making personal attacks or using emotionally charged language to avoid unnecessary disputes.
