All requests for use of samples and/or data from the STH-ObS Research Tissue Bank must be reviewed by the STH-ObS Scientific Advisory Board (SAB). Applicants should complete the form below including acknowledgement of the terms at the end of the form. After submission the research team will arrange for SAB review, and we aim to make a decision on all applications within one calendar month from submission.

Once sample release is approved by the SAB, the institution(s) receiving samples and/or data will be required to sign a Material Transfer Agreement (which will be forwarded) prior to shipping of any samples or data.
