Evidencing need for the My Ally service
All information is anonymous and only accessible by the Head of Service and the Evaluator. This information will be included in a final report to the project funder - ideally we want to evidence that there is a need for the service. This information will be used solely to report back to the funder of the service and all information will remain confidential. This form does not collect any contact information, names or email addresses - just the words you type into the boxes.
