ACAM Reference Form

This form is to be completed by a person who knows the ACAM applicant but is not a family member. A good example for a reference might be a coach, teacher, employer, or other community member who can speak to the applicant’s skills and character.

Once completed please return the reference form to the applicant in a signed and sealed envelope. The applicant is to submit this signed and sealed envelope with their completed application. You may also forward the reference letter to Thornlea Secondary School “Attention ACAM” or email your reference to Mme Ilana Bern (Head of Arts, Drama and Music at Thornlea), ilana.bern@yrdsb.ca with the subject heading “Attention ACAM REFERENCE.”
