In consideration of being allowed to participate in activities and programs of Spa 23 Fitness and Lifestyle hereinafter "Spa 23" and to use its facilities, equipment and machinery in addition to the payment of any fee or charge, I do hereby waive, release and forever discharge Spa 23 and its officers, agents, employees, representatives, executors, and all others from any and all responsibilities or liability for injuries or damages resulting from my participation in any activities or my use of equipment or machinery in the above-mentioned facilities or arising out of any participation in any pool activities at said facility. I do also hereby release all of those mentioned and any others acting upon their behalf from any responsibility or liability for any injury or damage to myself, including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf or in any way arising out of or connected with my participation in any activities of Spa 23.
I certify that myself/ my child is in good health and has no medical conditions that would prevent them from participating in physical activities, including swimming.
I understand that it is my responsibility to ensure that myself/ my child wears appropriate swim attire and uses any provided safety equipment, such as life jackets, while in the pool area.
I acknowledge that the health club staff reserves the right to remove any participant from the party if they are not following safety rules or if their behavior poses a risk to themselves or others.
I release and hold harmless Spa 23, its owners, employees, and representatives from any and all claims, liabilities, damages, or expenses arising out of my child's participation in the party, including but not limited to personal injury, illness, or property damage.
In the event of an emergency, I authorize the health club staff to obtain necessary medical treatment for myself/my child and agree to be responsible for any associated medical expenses.
Cellphones are not permitted in the locker room area. As per each event, the club can exempt cell phone use during event.
As a parent, I am delighted to grant permission for my child's image to be used for social media and advertisement purposes. I trust that any such usage will be conducted responsibly and with respect for my child's privacy and well-being. Thank you for considering my consent in this matter.
I have carefully read and fully understand the terms of this waiver. By signing below, I voluntarily agree to be bound by its terms and waive any right to bring legal action against Spa 23 Fitness and Lifestyle for any injuries or damages sustained during the party.
Please note: This waiver must be signed by a parent or legal guardian prior to the child's participation in the party.
