4S/4S Field Trip Chaperone Form

Students have two upcoming field trips. We will be going to the Biomes Marine Biology Center on Wednesday, May 29th. That is a full day field trip. We will then be going to the Luther Store Museum on Wednesday, June 5th. That is a half day field trip in the afternoon. Information and permission slips for each field trip will go home two weeks prior. However, we are looking for volunteers who are interested in chaperoning.

We are allowed to take 5 chaperones with us on our Biomes Marine Biology Center field trip and 2 chaperones to Luther Store. If you would like to be a chaperone, you must be CORI checked and fingerprinted. If you have already completed these requirements and are available on one of these dates, please fill out this Google Form by Wednesday, May 1st. We will randomly select our chaperones on Friday, May 3rd, and you will be notified if you are chosen.
