Two nominees from each undergraduate institution are eligible for consideration for the TAAHP Achievement Award. A nominee should be a student who has matriculated to an allied health profession program during the 2013-14 academic year. The student selected to receive the award with receive a $1000 cash award. In addition to completing this form, the TAAHP undergraduate pre-health adviser must email:
(1) a copy of the nominee's unofficial undergraduate transcript (2) a letter of support detailing the reasons the adviser believes the student is a good candidate for the award (3) a one page personal statement written by the student explaining their personal and academic experiences that motivate them to pursue their professional goals.
All nominated students will be recognized at the annual TAAHP meeting in February. Please send your documents to support your nomination to Cynthia Powell at firstname.lastname@example.org.
All nominations must be submitted by December 7, 2013.
