Please complete the following form for EACH SCHOLAR enrolled at Rocky Mount Prep and indicate whether or not you intend for them to return during the 2025 - 2026 school year.

The Deadline to return or complete this form is Friday, January 17, 2025.

If the form is not completed by January 17th, we WILL NOT be able to guarantee your scholar a seat for the 2025 - 2026 school year and you will need to fill out an application in order to return.
