Consultation Form Development Strategy of the Center for Initiatives for Society Foundation to 2030

Dear Friends of the FCIS Foundation – Members, Associates, Volunteers, Partners, and all recipients of our missionary activities,

After 15 years of operation, the FCIS Foundation has taken on the challenge of adopting a multi-year development strategy with new goals, a vision for development, and innovative social service projects.

We are an organization founded on strong social values and close to people.

We invite you to consult our Strategy to 2030. Therefore, please complete a short survey.

The mission by 2030 is: "Creating innovative social solutions that improve the quality of life of residents in terms of health, education, and ecology, and increasing development opportunities for people from groups at risk of social isolation and requiring special social support."

The vision by 2030 is: "Creating a space and managing a network of social services called Podlaskie Therapeutic Gardens and introducing internationally innovative solutions in Poland that facilitate the functioning and development of every person."

3) Your personal data will be processed pursuant to Article 6(1)(b) of the GDPR
for the purpose of performing this contract;

4) Your personal data will be transferred only to entities authorized by law;

5) Your personal data will be processed until the purpose of processing ceases to exist or for the period resulting from the archival category of the documents in which the data is included, specified in the implementing provisions of the Act on National Archival Resources and Archives.

6) You have the right to request from the Controller:

a) pursuant to Article 15 of the GDPR, access to your personal data, rectification, erasure, or restriction of processing,

b) pursuant to Article 21 of the GDPR, objection to processing,

c) pursuant to Article 21 of the GDPR, data portability,

d) withdrawal of consent to data processing, without affecting the lawfulness of processing based on consent before its withdrawal,

7) In the event of a personal data breach, you have the right to lodge a complaint with the President of the Personal Data Protection Office.

8) Providing personal data is voluntary; however, failure to provide the personal data required by the Controller may result in the inability to participate in a public task.
