This Waiver, Release of Liability, and Parental Consent Agreement ("Agreement") is entered into by the undersigned parent or legal guardian on behalf of the participant identified below in connection with participation in Coastside Basketball programs, clinics, practices, games, camps, tournaments, and related activities conducted by or in partnership with Coastside Basketball, John Parsons, the Boys & Girls Club of the Coastside, and their respective officers, directors, employees, volunteers, coaches, agents, sponsors, affiliates, and representatives (collectively referred to as the "Released Parties").

I understand that participation in basketball and related athletic activities involves inherent risks, including but not limited to:

• Falls and collisions
• Sprains, strains, fractures, and dislocations
• Head injuries and concussions
• Illness or medical emergencies
• Injuries resulting from facility conditions, equipment, or other participants
• Permanent disability or death

I knowingly and voluntarily assume all risks, known and unknown, associated with my child's participation in Coastside Basketball activities.

In the event of an emergency and if I cannot be reached, I authorize Coastside Basketball, John Parsons, the Boys & Girls Club of the Coastside, their coaches, staff, and volunteers to arrange for emergency medical treatment for my child.

I understand that I am responsible for all medical expenses incurred as a result of such treatment.

To the fullest extent permitted by law, I, on behalf of myself and my child, hereby release, waive, discharge, and hold harmless the Released Parties from any and all claims, demands, actions, causes of action, damages, losses, liabilities, injuries, costs, or expenses arising out of or relating to participation in Coastside Basketball activities.

This release includes claims arising from ordinary negligence of the Released Parties but does not apply to gross negligence, reckless conduct, or intentional misconduct where prohibited by law.

I certify that I am the parent or legal guardian of the participant named above and have the legal authority to execute this Agreement on the participant's behalf.

I understand the nature of the activities involved and grant permission for my child to participate in all Coastside Basketball activities.

I acknowledge that participants are expected to demonstrate sportsmanship, respect, and appropriate behavior toward coaches, officials, volunteers, teammates, and spectators.

I understand that failure to comply with program rules may result in disciplinary action, including removal from participation without refund.
