    GENERAL PROGRAM REQUIREMENTS

    The Title IV-E Stipend Program requires that students complete all the following: 1. Maintain academic good standing during full-time or 3-year (part time) enrollment in the MSW Program, a 3.0 G.P.A or better every semester and receive credit for satisfactory performance in the field practice. 2. Successfully complete and receive a grade of 'CREDIT' in two years of field internship placement. One year of placement will take place in a county public child welfare agency. 3. Maintain automobile insurance while enrolled in internship in the MSW Program. 4. Attend all child welfare trainings seminars, and meet the MSW Title IV-E Child Welfare Curriculum Competencies. 5. Applicants must agree to work in a public child welfare agency (CWS) in California for at least 2 full years upon graduation. 6. County Letters of Support* a. MSW Title IV-E 3-Year (part time) Program: If you are applying to the Title IV-E 3-Year (part time) program, you must be currently employed by a County Social Services Agency, and a letter of support from the Director of the agency is required for your application to be considered. Letter of Support is due with application or prior to your awards interview. b. MSW Title IV-E Full time Educational Leave of Absence: If you are currently employed by a County Social Services Agency and are planning to take a leave of absence from your employment to attend graduate school (Educational Leave of Absence), a letter of support from the Director of the agency is required for your application to be considered. * Letter of Support must be submitted to the Title IV-E Program prior to or upon acceptance to the MSW program and the Title IV-E Program. Applicants will not be awarded a IV-E stipend unless a Letter of Support has been received by our office. 7. Criminal Background Check (all IV-E students): Applicants must undergo pre-screening for county employment (including fingerprinting, background and criminal record checks) and be found eligible for county employment.     8. Title IV-E Employment Payback Obligation: Upon graduation, the Title IV-E Child Welfare Training Program requires students to seek and accept employment in a public child welfare agency within the State of California. If necessary, are you willing and able to relocate within the State of California in order to meet your Title IV-E Program work requirements?

    1. Statement of Intent: A 2-3 page typed statement explaining your understanding of public child welfare services. Also, address your interest in and commitment to a career in public child welfare and your view of how this stipend program relates to your short-and long-term career goals. 2. Resume: A 1-2 page typed resume, including a list of your relevant paid and volunteer experience.

    1. I hereby attest that I have never been convicted of a felony crime or any crime involving harm to children. I hereby attest that I have never been discharged from employment at a county or other social services agency due to violation of county code/merit system rules or violation of agency or professional codes of ethics. 2. If granted the Title IV-E Master of Social Work Child Welfare stipend, I hereby agree to adhere to the requirements of the contract. 3. Further, I certify that the statements given in this application are true. Further, I hereby release to the SJSU Title IV-E Awards Committee the information contained in my application to the MSW Program at the School of Social Work at San Jose State University.

    IMPORTANT DATES TO REMEMBER

    - Monday, January 27, 2014: Title IV-E Informational Meeting at the School of Social Work Office (WSQ 215) at 3:00 PM - 5:30 PM, Large Conference Room - Friday, March 14, 2014: Title IV-E Applications deadline by 5:00 PM - Mondays, April 14, 2014 & April 28, 2014: Stipend Awards Interviews (BY INVITATION) Applicants will be notified by Thursday, May 15, 2014 if they have been selected to participate in the IV-E Program and receive a stipend.
