New Students Record Request Info 2024-2025
Students who are NEW to Box Elder School District will need to fill out the following form in order for us to request your records from your previous school. BEHS will be requesting a transcript, birth certificate, immunization record and an IEP, ELL or 504 plan if applicable.
Once ALL of these records have been received, you will be contacted to set up an intake appointment. You MUST bring proof of residency and the fee payment of $116 or a completed fee waiver application and proof of income with you to your appointment. Registration appointments will begin after August 6th.
