General Release of Liability Waiver

I, as the parent/guardian of the player, understand that ice hockey training involves risk of injury. I accept all risks on behalf of the player and agree not to hold Advanced Hockey or BK Hockey and its owners responsible for any injury, loss, or damage that may occur during these programs (on or off the ice), except where prohibited by law. I confirm the player will only participate if medically fit and I consent to emergency medical care if needed. I agree to follow program rules and facility policies.
