2. Once there, under "HOW MUCH WOULD YOU LIKE TO GIVE?" please click OTHER and enter the donation amount (listed above) for your participation level.

3. Under "DESIGNATION" please click "GOLF EVENT ."

4. Click DONATE.

5. On the second screen, please fill in your personal information and credit card details.

6. Under "COMMENTS", please type in the participation level (from above) that you are choosing there.

7. Click "CHECK OUT"

A copy of your responses will be emailed to the address you provided.
