In this next section you will complete each applicable item to describe your plans post-graduation. You will need to provide the name of the college/university/trade school and the semester you plan to start (i.e. Fall, Winter, etc,), as well as the subject you plan to major in or the certification program. (This information is mandatory. Your application will not be considered if this is  not completed).

List the name, email and telephone number of THREE (3) people who have first hand knowledge of your academic abilities and/or drive and personal character, and who have agreed to write a recommendation for you. Two of the references must be a full-time faculty member at your school (another acceptable source can be your direct work supervisor). The remaining reference can be a close friend or family member. All recommendation letters must be sent directly from the reference to the following email address: Jimsway@truheart.org
