This is a form whereby parents, children, coaches, and community members can file official complaints with our organization that will go on record in our board meetings. Each complaint will be reviewed and discussed by the board and an official decision on action taken will be voted on by the board.
***Please note that everything you write on this official complaint form will be included in our board's official files, with minutes of the meeting in which we discuss them. Once the board has reviewed your complaint and made a decision about our course of action, we will contact you.
****If you wish to remain anonymous from the person/people you are filing the complaint against, please note that on this form right after your name. If so, we will keep your name/contact private when we discuss the situation and in our meeting minutes. If you remain anonymous the only person who will see your name/contact information is our President. If your complaint is against our President and you wish to remain anonymous from her, please contact another board member. Thank you!
