Thank you for taking the time to apply to be a model family for Alimegreendream. Please allow a couple of minutes to fill in this form. The more detailed your information the better I get to know you and the more chance you have of being selected. Any personal information you share will be kept safe & secret and won't be shared with anyone else.

The photos & video from this session will be used for the purposes of creating a film portfolio. In exchange for participating, you'll receive some photos and a short film from your session. You must be willing for your photos & video to be used for this purpose, and for any marketing purposes on my website (www.alimegreendream.com) and social media as well.
