Please complete this report out and return it to Karen Chasse in the Leadership Office. The Foundation looks forward to learning about the grant’s outcome. This reflection is not evaluative of you or your teaching but an objective description of
the funded program’s success, failure, or mediocrity.

As indicated on the grant application form, it is critically import that we receive photos or a video of the grant in action in conjunction with this reflection. These materials will aid the Foundation in sharing successful projects with the community. When appropriate, final projects, reflections and/or thank you notes from students would also be appreciated.
Because such documentation may be used for website and/or in other types of media, prior to submission, you must check with your school’s secretary/administrator to ensure that the subject or author of any such documentation has granted 'permission to publish'.
