In Consideration of participating in the 2024 Zombie Shootout Lacrosse Tournament,

I acknowledge and understand that my child will be engaging in an activity that involves the risk of serious injury including permanent disability or death as well as severe social or economic losses.
I hereby agree for ourselves, our heirs, executors and administrators, to release, hold harmless and forever discharge covenants to indemnify and not sue Bethe Park Boys Lacrosse Booster Association, their affiliated organizations and sponsors, their coaches, managers, employees and associated personnel, officers, members of the Board of Directors, agents, including the owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as "releasees," from any and all liability to each of the undersigned, his/her heirs or next of kin for any and all against any claim by or on behalf of the applicant as a result of the applicant's participation in the 2024 Zombie Shootout and/or being transported to or from said event.
I knowingly assume all risks associated with my child's participation in the Tournament, even if arising from the negligence of the participants or others and assume FULL responsibility for my child's participation in the Tournament.
I certify that my child is in good physical condition and can participate in this Tournament.
Further, I authorize the tournament director to request medical treatment as necessary to insure my child's well-being.
I am voluntarily submitting player and guardian information.
Further, ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT in consideration of being allowed to participate in the Bethel Park Zombie Shootout and related events and activities, the undersigned acknowledges, appreciates and agrees that participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROMT HE NEGLIGENCE OF THE RELEASEES or others and assume full responsibility for my participation.
I agree that all photographs, either still or moving, taken during participation in the Zombie Shootout may be used without limitation for publicity, promotion and advertising of the Tournament.
