Lasley Elementary: Hybrid or Remote/Online Learning Preference for 6th Grade
To plan for the reopening of our school in August, we need all of our families who have students enrolled in grades 6th grade to fill out this form for each child coming to our school this year.  

You need to complete one form for every sixth grade child at our school. When you finish filling out the form for your first child, you can click on a button at the end if you need to fill it out again for additional children at our school.
