Parents Day Out registration
Days of Operation: Wednesdays/Fridays 9:00 a.m.-1:00 p.m. (Following Fayette County Public School Calendar). Start date will be Wednesday, September 5, 2018, with the last day being Friday, May 17, 2019.

Tuition: $50/WK per child. Child must attend both days (at this time we are unable to offer a one-day option). A $75 non-refundable annual materials fee is due with this form. (See “Enrollment” below).

Crestwood offers an online capability to make payments, either as a recurring event or not one-time, using credit/debit cards or your checking/savings account as an ACH transfer. This can be used for PDO, for registration fee and/or tuition payments. When providing the information, please ensure that you specify the purpose of the payment as PDO registration or PDO tuition.
