UVSS Food Bank Student Hamper Program
Please read the information below before filling out the form:

PLEASE DO NOT FILL OUT THIS FORM MORE THAN ONCE A WEEK. If you want to continue receiving hampers, we recommend filling this form out again the same day you are scheduled to pick up your first hamper.

Students are able to request a hamper once a week but must fill out this form again each week. Upcoming dates for hamper registration and pick up (pick-up dates are dispersed throughout the week):

Registration is due by  Friday, Sept 29th at noon (12pm) for pick-up the following week (Oct 3rd-6th)
               
You will receive an email from us the Friday, Saturday, or Sunday after registering with your pickup date and time. Please check your spam to ensure you don't miss a confirmation email from us!
