After School Daycare Program Enrollment
After School Daycare Program is for students in K4-5th grade.  It will run from the end of the school day (approximately 3:25) until 5:45 PM, Monday through Friday on days that school is in session (Please note there is a $1.00/minute per student late fee that will be charged for every minute you are late in picking up your child(ren).  Families must complete this form to register your child(ren) to attend the After School Program each year.  The cost of the program is $10.00 per child per day.  Statements will be mailed out on Monday and payment is due to the office by Friday.  Services will be cancelled if payment is not received after two weeks.  Students will be provided a snack, exercise and physical activity along with homework help and tutoring.  We will only bill for services provided.
