The purpose of this class project survey is to examine the effect of covid-19 and how it has impacted students, teachers, and professors. We are also interested in examining how people believe covid-19 has impacted those of diverse backgrounds.

I understand that I will be asked to answer demographics questions and questions about gender, self-esteem, and covid-19. I understand that there are no risks associated with participating in this research and that my participation in this study should take a total of five minutes.

I understand that my responses will be confidential, and that no identifying information will be collected, and that all data will be stored in a password protected electronic format. I understand that the results of this study will be used for scholarly purposes only. I know that my participation in this study is voluntary. I may refuse to answer any question asked and may discontinue participation at any time. I also understand that I will not be penalized if I decide not to participate in this study or if I withdraw participation. Potential risks resulting from my participation have been described to me.
