Please complete the form to receive breakfast and lunch delivery/pick up for children in your household during the 2020-21 school year while we are in a remote/hybrid model.  Food will be available for pickup for students that do not require delivery services at the 209 complex, HBE and Middle School on Tuesday and Thursday from 10:30am - 12:00pm.  Each delivery will provide meals for the whole school week. Any child ages 2-18, will be eligible for either option. The delivery service is to accommodate students in this time of Remote/Hybrid Learning.  

Meal Distribution Model:
- Tuesday delivery/pickup: Lunch (for Tuesday and Wednesday); Breakfast (for Wednesday and Thursday)
- Thursday delivery/pickup: Lunch (Thursday, Friday, and Monday); Breakfast (for Friday, Monday, and Tuesday)

Please be aware that someone is required to be home at the time of delivery in order to receive the food items. The estimated time of delivery will be between 10:30am and 12:30pm.
