Baltic Updated Meal Count Form

There has been a change to the Food Service guidelines.  We are now able to serve meals to any child 0-18 years old regardless of their enrollment in our school district.  If you are currently getting meals and filled out the previous meal survey and you are not adding any new children to the list, please do not fill out this survey.  We only need people to fill this new survey out if they are adding new children to their previous request or if you are adding new students in general.

Example:  On the previous survey, we had requested four meals.  We now want to add one more meal for our three year old child.  We would only request one meal on this new survey.

The lunch meal will include a ham and cheese sandwich or turkey and cheese sandwich, a whole fruit, a bag of chips,  a packaged bag of available vegetable and small side salad (based on inventory), a white or chocolate milk, (choice based on inventory), and a cup of ranch, a packet of mayonnaise, and a packet of mustard.  The breakfast meal will include a Pillsbury pastry, whole fruit, milk and juice.

The meals will be available for pickup at the school from 11:30am-12:30pm daily. At this time, you will be given lunch and the next day's breakfast, there is no separate breakfast pickup. If your family is not able to pick up the meals at school, arrangements will be made to deliver them within the confines of the City of Baltic and current rural bus routes.  So that the correct number of meals can be prepared, please fill out the form below.
