Columbia Montour Chamber of Commerce Awards Nomination Form

A panel of volunteer judges will select a recipient in each category. The awards will be presented at the 2026 Annual Meeting Ceremony. The Awards Committee reserves the right to withhold awards in any category should they feel nominees do not meet the award criteria. Nominees will be advised of their nomination before the Annual Meeting Ceremony. When submitting nominations please provide as much detail as you are able about the nominee. This application is the only information the awards committee considers in their selection process.
