Filling Out This Registration & Waiver
Our new electronic Registration & Waiver is in three parts: our Release & Waiver; our Rules & Policies; and our Student Registration Form. Please read through the first two sections and check the agreement boxes; then fill out the Student Registration Form completely. A red asterisk indicates a required answer. A separate registration form must be filled out for each student.
New York Gymnastics Center Release & Waiver
Participant, in attending the gym and using the facilities, does so at their own risk. The gym operator shall not be liable for any damages arising from personal injuries sustained by participants in or about the premises. Participant assumes full responsibility for all injuries and damages which may occur in or about the premises and he or she does hereby fully and forever release and discharge the gym operator, all associated gyms, their owners, employees, and agents from any and all claims demands, damages, right of action, present or future, resulting from or arising out of the participant's use of the gym and/or its facilities. Participation is entirely his or her own choice and with the understanding of the risk of accidental injury involved in any activity involving unusual motion or height. If a minor, the parents acknowledge that they know of this injury risk the minor is assuming. I acknowledge that the risk of injury does exist in the activity of gymnastics or similar and understand that risk is my (child's) own.

TUITION PAYMENTS
Payments must be made for the complete three- to four-month sessions (or the remaining portion when enrolling). Payment can either be made by credit card in full or in monthly installments. Monthly installments must be made through a credit card authorization form (available on nygymnasticscenter.com or in the gym) or through post-dated monthly checks.

CONTACT INFORMATION
WE COMMUNICATE IMPORTANT INFORMATION THROUGH EMAIL, including gym closures, monthly emails, and student information. Please make sure this registration form includes an email that is checked with frequency.

MISSED CLASS POLICY
We do not allow make-up classes. Tuition is paid for a spot in a class, on a day, at a specific time. If you miss a class, we cannot fill that empty spot with another student. Usually, all of our classes are full, thus we cannot allow make-up classes for any missed classes. Please try to make all of your scheduled classes. NO REFUNDS ARE GIVEN FOR MISSED CLASSES.

STUDENT BEHAVIOR
If your student behaves in any manner that is disruptive to the class/lesson, disrespectful to staff or other students, or behaves in any unsafe manner, they will be sent to the seating area for (all or a portion) of their current class, and parents will be notified. If this happens a third time, the student will not be allowed to return to our facility. NO REFUNDS WILL BE GIVEN FOR STUDENTS WHO ARE REMOVED FROM OUR FACILITY FOR BEHAVIOR ISSUES.

WHAT TO WEAR
  • Sandals, Crocs, or easy shoes to slip on/off at door cubbies
  • Flexible clothing (t-shirt/shorts, leotard, warmups, etc.)
  • Bare feet (socks allow slipping on most events)
  • NO hoodies or jewelry of any kind
  • Long hair must be tied back

WATER BOTTLES
Students are welcome (not mandatory) to bring water bottles for water breaks. We have filtered water on site to fill. However, students are only allowed to drink from those water bottles, before class, after class, or during our water breaks.

EXCEPTIONS TO THESE RULES AND POLICIES
These rules and policies are required for a safe and comfortable learning environment. For exceptions to any of the stated rules, please feel free to email Mr. Mike (nygymcenter@gmail.com) and discuss your situation.
