Please fill out this order form if you would like to order breakfast and lunch for your child(ren) starting Tuesday, Sept 29th. If your child is enrolled in the Park District PALS Program please fill out this survey as well. PALS meals will be delivered on site.

Understanding it is essential all children have access to nutritious food,  we are excited to announce the United States Department of Agriculture has extended food service waivers; therefore, children under the age of 18 are qualified to receive individually packaged breakfast and lunch meals at no charge.

  Meals can be picked up curbside from Middle School South or delivered via bus to a local designated stop between the hours of 8am-10am.

Pick up and deliveries will be Mondays and Wednesdays unless Monday is a Holiday delivery or pickup will be Tuesday and Wednesday.

 Moving forward we will no longer being sending out a survey every two weeks seeing that all students are eligible for this program. If you sign up you'll remain on the pickup or delivery list unless you contact our district office. Ongoing pickups will be on Mondays and Wednesdays without having to fill out any other surveys.  

In addition, families who have registered to have their meals delivered will be contacted.
