ALC PI/Project Form
FILL THIS FORM ONLY IF YOU ARE PI OF AN ALC SESSION

This form is designed to gather information information about PIs/projects that benefit from ALC activities, which NSF requires us to report each year. The information we gather will only be reported to NSF as per funding requirements, and will not be released to other sources. Information is only provided to NSF in aggregate and is entirely anonymous (i.e., no names of visitors are reported).

If your research is funded by NSF, we need to report the EAR numbers and titles of the NSF awards that support your research. This information is used by NSF to determine which programs have supported ALC activities.

If your research is not funded by NSF, we need to report only general information about your project (e.g., title of the study, source of support, etc.) and your professional level and institution. 

Demographic information is only provided to NSF in aggregate form and in a completely anonymous fashion (i.e., no names are reported).

Please complete this form for each project that involves analyses conducted at ALC.
