Membership Cancelation Form
If you wish to cancel your membership with us, we ask that you please fill out and submit our form below.

Reminders:  
  • In the event of injury or hospitalization, memberships can be frozen or extended. Please speak with the Front Desk regarding these arrangements. 
  • Memberships can be frozen for two months, for any reason, 1x per year.
  • Cancellation requests must be made at least thirty (30) days prior to the month in which it will take effect.
  • If you miss the thirty (30) day notice deadline, you will be responsible for the following month’s membership dues.

Please fill out and submit the form below to request your membership cancellation.  You will receive a confirmation within forty-eight (48) hours.
