I understand that my information will be a part of a Research project; this information may be put in a paper that is submitted to AP Collegeboard and will be seen by teachers. I consent to whatever I submit to be put in this research paper, and I will still remain anonymous with that.
I will not submit personal/sensitive information about myself including:

- Addresses
- School Name
- City, County, State, or Country
- Any explicit/vulgar material
- Any other sensitive or identifying information

Me, and my parents consent to putting this information into this form. I am over the age of 13 and I understand that the information I submit will be shared to AP Collegeboard and may be used for the purpose of an AP research class. I will still remain anonymous even when I submit this form.
