Fire It 3v3 MITE: Player Registration Form
Please fill out the information below about your player. Info will be kept on file to ensure each week the teams are split as fair as possible in regard to age/ability. You only need to fill this out once. Please fill out one per player. Ages are 2016 and under for the 2024-25 season. Players older than 2016 birth year will not be permitted to play, regardless of skill level. 

Disclaimer: This is NOT a learn to skate/ learn to play. There are no coaches instructing and there is no referee. Hat Trick Ice Arena is a miniature ice rink with boards and regulation size nets (big nets).The game will be run by the owner of Fire It; who is a USA Hockey U8 Coach with all required certifications for the 2024-25 season. Fire It is not directly associated with any team or organization. 

For players:  Players will play 3v3 for 2 minutes each line for the 40 minute game. Players will change on the whistle. Skills to develop will include quick decision making, small area battles, quick passing/shooting and crashing the net. There will be 9-12 players on each team depending on the week. 

For Goalies: These are high scoring games, so they need to understand that going into it. The skills to be worked on are quick feet, angles, reaction time and rebound control.

By participating in Fire It 3v3 at Hat Trick Ice Arena, each player (and their parent/guardian if the player is a minor) acknowledges and agrees to the following terms and conditions:

1. Assumption of Risk: Participation in ice hockey involves inherent risks, including but not limited to physical injury, falls, collisions, and other accidents. By participating in this program, you voluntarily assume all risks associated with ice hockey activities.

2. Waiver and Release: In consideration of being allowed to participate in Fire It 3v3 Hockey League, you hereby release and discharge Hat Trick Ice Arena, Larisa Cavallaro, and all of their respective employees, agents, representatives, and affiliates from any and all claims, liabilities, or causes of action arising out of or connected to your participation in this program. This waiver and release include, but are not limited to, claims arising from negligence.

3. Medical Treatment: In the event of an injury or medical emergency, you consent to the provision of first aid and any medical treatment deemed necessary. In the event of an emergency, parents in attendance should coordinate someone calling 911 and getting the ambulance to the facility. You further agree to bear the costs of any such treatment.

4. Compliance with Rules: You agree to abide by all rules and regulations established by Fire It 3v3 Hockey League and Hat Trick Ice Arena. Failure to comply with these rules may result in your removal from the game and future games without a refund.

5. Indemnification: You agree to indemnify and hold harmless Hat Trick Ice Arena, Larisa Cavallaro, and all of their respective employees, agents, representatives, and affiliates from any and all claims, liabilities, damages, or expenses, including attorney's fees, arising out of or connected to your participation in this program.

6. Photography and Media Release: You grant permission for Fire It 3v3 Hockey League to use any photographs or video footage taken during the program for promotional purposes without compensation.

By agreeing below, you acknowledge that you have read and understood this liability disclaimer, and agree to be bound by its terms. 

You must be at least 18 years old and the legal guardian of the player to acknowledge the liability below.

  • Players (Please review with your player prior to signing)

    Be Kind and Respectful

    • Treat all players, parents, and Fire It representatives with kindness and respect.
    • Use polite words and actions on and off the ice.
  • Play Fair

    • Follow the rules of the game.
    • No cheating or taking unfair advantage of others especially due to age or ability.
  • Try Your Best

    • Give your best effort in games, it's fast paced and you will be tired. You get out what you put in to it!
    • Be a good sport whether you win or lose.
  • Stay Safe

    • Always wear your hockey gear (helmet, pads, etc.) while the game is in progress and you are on the bench or ice. Neck guards are mandatory.
    • Follow instructions from Fire It representatives to avoid getting hurt.
  • Have Fun

    • Enjoy playing hockey and encourage your teammates and members of the opposing team.
    • Remember that the goal is to have fun, make friends and work on your hockey skills.
    For Parents and Guardians:
    1. Support Positively

      • Encourage your child and all players with positive comments.
      • Avoid yelling at or criticizing players, other parents or Fire It representatives.
    2. Respect the Program/ Facility

      • Follow the rules and guidelines set by Fire It and the rink.
      • Ensure your child arrives on time.
    3. Model Good Behavior

      • Set a good example for your child by showing respect and sportsmanship.
      • Help create a positive and supportive environment for all players.
    4. Communicate

      • Share any concerns or questions with the coaches or league organizers in a respectful manner.
      • Inform the league of any special needs or medical conditions that may affect your child’s participation.

    Failure to comply with the Code of Conduct will result in a series of consequences designed to ensure a safe and positive environment for all participants. The first instance of non-compliance will result in a verbal warning from a Fire It representative. If the behavior continues or is of a serious nature, it will lead to a temporary suspension from the current and/or future games or activities. In such cases, the player may not be allowed to participate for the remainder of the current game or a series of future games. The duration will be up to Fire It's representatives. 

    Should violations persist post suspension, a meeting will be arranged with the player's parents or guardians to discuss the behavior and develop solutions to prevent further incidents. Repeated or severe violations may result in a permanent suspension from the program, meaning the player will no longer be allowed to participate in any Fire It games.

    If a parent or guardian fails to comply with the Code of Conduct, they may be asked to leave the facility and could face a temporary or permanent ban from attending events, which may also affect the player's participation in the league. Please note that in the event of any suspension, temporary or permanent, no refunds will be issued for any missed games or sessions. Our primary goal is to maintain a fun, safe, and positive environment for everyone, and we appreciate your understanding and cooperation in upholding the Code of Conduct.
