By participating in Fire It 3v3 at Hat Trick Ice Arena, each player (and their parent/guardian if the player is a minor) acknowledges and agrees to the following terms and conditions:

Assumption of Risk: Participation in ice hockey involves inherent risks, including but not limited to physical injury, falls, collisions, and other accidents. By participating in this program, you voluntarily assume all risks associated with ice hockey activities.
2.
Waiver and Release: In consideration of being allowed to participate in Fire It 3v3 Hockey League, you hereby release and discharge Hat Trick Ice Arena, Larisa Cavallaro, and all of their respective employees, agents, representatives, and affiliates from any and all claims, liabilities, or causes of action arising out of or connected to your participation in this program. This waiver and release include, but are not limited to, claims arising from negligence.
3.
Medical Treatment: In the event of an injury or medical emergency, you consent to the provision of first aid and any medical treatment deemed necessary. In the event of an emergency, parents in attendance should coordinate someone calling 911 and getting the ambulance to the facility. You further agree to bear the costs of any such treatment.
4.
Compliance with Rules: You agree to abide by all rules and regulations established by Fire It 3v3 Hockey League and Hat Trick Ice Arena. Failure to comply with these rules may result in your removal from the game and future games without a refund.
5.
Indemnification: You agree to indemnify and hold harmless Hat Trick Ice Arena, Larisa Cavallaro, and all of their respective employees, agents, representatives, and affiliates from any and all claims, liabilities, damages, or expenses, including attorney's fees, arising out of or connected to your participation in this program.
6.
Photography and Media Release: You grant permission for Fire It 3v3 Hockey League to use any photographs or video footage taken during the program for promotional purposes without compensation.
By agreeing below, you acknowledge that you have read and understood this liability disclaimer, and agree to be bound by its terms.
You must be at least 18 years old and the legal guardian of the player to acknowledge the liability below.
