I acknowledge that this activity entails known and unanticipated risks which could result in physical or emotional injury, paralysis or death, as well as damage to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity and hereby agree to protect, defend, indemnify and save harmless and reimburse Algoma School District, Algoma Venture Academy, its Governance Board, officers, agents, and employees (hereinafter referred to collectively as the School District) from and against any and all loss, claims, lawsuits, liability, expenses and attorney’s fees and costs of any kind and nature whatsoever, which the School District may incur arising out of or in connection with this activity.

I certify that I/my child have no medical or physical conditions, as noted above, which could interfere with my/my child's safety in this activity and I authorize qualified emergency medical professionals to examine and in the event of injury or serious illness, administer emergency care. 

In the event it becomes necessary for the School District/staff member to obtain emergency care for myself/my child, neither the staff member nor the District assumes financial liability for expenses incurred because of the accident, injury, illness, and/or unforeseen circumstances.
