If a student or staff member needs to see the nurse this form must be completed in order to receive an appointment. Students/Staff are not permitted to go to the sick area of the clinic unless it is an emergency.  The nurse will prioritize visits and notify the student or staff member when the nurse is first available to see them via google email.

Prior to completing this survey for a clinic appointment, staff should make every effort to provide support through resources included in the self-care station located outside the nurses office.  

**If a student or staff member needs to be seen immediately, do not hesitate to contact the nurse to get immediate medical assistance.
