USA Hockey Age Waiver Form (Youth to Adult or Adult to Youth) is only required to be submitted for review and approval to the Sled Hockey Fair Play Committee (FPC) if:
1. A player is over the age of 18 but does not possess the skill level required to play with an adult program at any level.
2. A player is under the age of 18 (as of December 31st) and will be at least 15 (as of December 31st) who possesses the required skill level to play in an adult program.

All waivers are due to the FPC a minimum of three weeks before an event. The FPC will review Age Waiver requests starting September 1st through December 31st for the respective season. Waivers received after December 31st will only be reviewed under exigent circumstances, and at the sole discretion of the FPC. The FPC will not review ANY waivers the week of December 25th to January 1st. Any waivers received during that time will be reviewed as a part of the final cycle of the new year. If an event occurs during that time frame, waivers must be submitted three weeks before December 25th. Waiver Requests are reviewed on the Monday of each week, and a decision or follow up will be made by the end of that week. Waivers are only required for USA Hockey-sanctioned events (Classic, Nationals)
Instructions:
An age waiver can only be submitted by the player's home program. The person completing and submitting the form must fulfill a leadership role with the team/program. (ex: Coach or manager). Individual players will not submit their own waivers and if received, will be returned unreviewed.
If a youth player is waived onto an adult team for any USA Hockey sanctioned national event (Classic, Nationals), and that player makes the decision to return to their youth program, that player will no longer be allowed to roster on an adult team at a national event until they reach the age of 18.
Subsequently, if a youth player waives onto an adult team for a USA National event and continues to play adult following that event, they will no longer be allowed to roster for their youth program.
If a youth player is uncertain about their ability to play as an adult, they may roster onto an adult team for an in-season game to “test the waters”. That player will not be allowed to dual roster during that event or tournament.
If an adult player has been waived onto a youth team for 3 consecutive years, that player is grandfathered into the youth program and would not be required to submit a waiver going forward. If their program were to develop an age-appropriate option, then that player would play for their chronologically aged team.
