Please use this to document training has been completed. You should fill this out in front of the trainee to give them a chance to confirm they are confident to complete the task in future without supervision.

After explaining and demonstrating the correct procedure, please confirm the Students ability to connect/disconnect a trailer safely. (remembering the use of dogs if necessary, ride height, connecting appropriate cables, landing legs/draw bar stays)

Please confirm you have shown the trainee the importance of having the sealing surface of the hatch clean of debris so a good seal is created

Confirm you showed the trainee the belt and lanyard in the site hut? Reinforce this must be used if required to work within 1m of the pit / pond. *
Please confirm that you have shown the trainee where the rescue throw-rope is including the SOP on its use. *

Confirm the trainee has been correctly inducted into site (Induction questionnaire to have been completed at Symons Yard). *
Observe the Students ability to display safe driving skills (road positioning, speed, courtesy). *
Confirm you have discussed the importance of a thorough clean of the tank. If tanks are not rinsed thoroughly a Confined Space Entry must be completed which puts our people under unnecessary risk. *

After explaining and demonstrating the correct procedure to the Student, please confirm they are able to complete a full load/unload cycle task without supervision. *
Please ensure you have informed the trainee the hatch is to be left open when leaving the tanker parked *
Please make any comments you believe are necessary regarding the Student, both good and work on's
