1. Assumption of Risk I understand that my child will be participating in indoor play, group games, crafts, and winter-themed activities. I acknowledge that participation may involve risks such as slips, falls, bumps, minor injuries, or exposure to other children. I voluntarily assume all risks associated with my child’s participation in this event. 2. Release of Liability I hereby release, waive, and discharge Bayou & Bloom, its owners, directors, employees, volunteers, and representatives from any and all claims, demands, injuries, damages, or liabilities arising out of or connected to my child’s participation in this Parents’ Night Out event, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. Medical Treatment Consent If an emergency occurs and I cannot be reached, I authorize Bayou & Bloom staff to obtain necessary medical treatment for my child. I agree to be responsible for any medical expenses resulting from such treatment.
4. Photo/Video Permission I give permission for my child to be photographed or recorded for Bayou & Bloom’s social media or promotional use.
5. Food Acknowledgment I acknowledge that my child will be provided pizza, juice, and a sweet treat. I am responsible for notifying Bayou & Bloom of any allergies or dietary restrictions. The organization is not responsible for reactions or incidents resulting from allergies that were unknown or not disclosed.
6. Payment & Registration Policy I understand and agree that: • Filling out this form does NOT guarantee attendance. • Payment to Venmo @bayou-bloom is required at the time of registration to secure a spot.
• All payments are non-refundable, including for absences or changes in schedule.
7. Pick-Up Policy I agree to pick up my child promptly by 8:00 PM. I understand that late pickup result in an additional fee of $10 for the first 5 mins and $1/ minute thereafter and denial of future participation. I agree that the same person who drops off is also required to pick up, unless discussed ahead of time or listed as the emergency contact . If a person other than the person who dropped off is picking up, a STATE ID is required and must match the name.
8. Behavior & Safety If my child’s behavior poses a safety concern to themselves or others, I may be contacted for early pickup. Bayou & Bloom may remove a child from activities if needed to maintain a safe environment.
