Have you have already tried to resolve your situation through 1- Early resolution and/or 2- A restorative conversation and/ or 3- Mediation? If you haven't and you feel that your situation can still be turned around with any of these options then please fully explore them first.  If you have not explored these options, we may recommend that you do so before a Formal Resolution is explored. 

However, if you have tried steps 1,2 & 3 and they still have not brought about the desired changes, please fill in this form with as much detail as possible on why you are requesting a Formal Resolution.  Once completed your request will come through to a private & confidential inbox that is only monitored by two people, once reviewed an appropriate person will be  in touch as soon as possible regarding the next steps.
