2020 Horizons Greens Farms Academy PreK-8 Teacher Evaluation
This student is applying for admission to Horizons GFA, a summer and school-year academic program working with students in PreK through college. The PreK-8 program is held for six weeks each summer on the GFA campus and is designed to help Bridgeport public school students build academic and social-emotional skills.

Students are selected regardless of academic ability. We seek students with demonstrated good behavior, the greatest potential to benefit from our program, and whose family is deeply committed to their child's education. We admit students from lower-income families only. Due to staffing limitations, we are unable to serve students with significant learning disabilities or behavioral issues.

Your evaluation of this applicant will be of great assistance to us. Please answer in a way that gives us as much information about the student as possible.
