This form may be used to request financial assistance in order for your child to participate in a school field trip or activity. This form may also be used to request aid with the purchase of school supplies and personal items such as backpacks, water bottles, and headphones. It is our goal that all students be able to participate and we have partners who can provide scholarship funds if needed.

This form will be reviewed by building administration, who will work to ensure discretion and confidentiality regarding your request. Student names will not be shared with any outside group. Once reviewed, you will be contacted by the school to confirm. We make every effort to protect student confidentiality in this matter. In support of this, you may choose to send an envelop containing a note or blank paper in leu of payment so that your child is not aware of any difference.
